PLEASE SCHEDULE AN APPOINTMENT
Once the online process has been completed and submitted, the student’s information will electronically be sent to the school. Please print ONE copy and sign your online documents.

(If you do not have access to a printer, school will print out copies for you.)

In order to complete the enrollment process, you will need to contact your school site office manager to set an appointment.

PLEASE BRING THE REQUIRED DOCUMENTS TO YOUR APPOINTMENT

PROOF OF RESIDENCY (Please provide TWO current documents from the following list)

  • Property tax payment receipts
  • Rental property contract, full lease agreement with signature page, or payment receipts
  • Utility service contract, statement, or payment receipts (gas, water, trash, or electricity only)
  • Pay stubs (must show name and residence address)
  • Voter registration
  • Correspondence from government agency (Social Services, DMV, Tax Returns, etc.)

If you are unable to provide two documents from the above list due to special circumstances, please contact the school office staff.

PROOF OF AGE (Please provide ONE of the following documents):

  • Birth Certificate
  • Statement by the local registrar or county recorder certifying the date of birth
  • Baptism Certificate
  • Passport

(If none of the options above are obtainable, an affidavit of the parent, guardian, or custodian, or any other appropriate means of providing the child's age) 

OTHER REQUIRED DOCUMENTS

  • Parent/Guardian Photo ID
  • Immunization Records
  • Physical Exam Report (Req. for Kinder & 1st grade only) 
  • Previous school information 
  • IEP or 504 Plan or GATE
  • Custody documents
  • Any medical forms provided by your child's doctor

Optional: (Items can be helpful in placing your child in the appropriate class or programs)

  • Last Report Card
  • Last School Attendance Record
  • State Test Results

Enrollment is not complete until all required paperwork has been submitted to the school office.

Notice

An enrollment determined to be secured by false, misleading, or deliberate omission of information to establish residency in the Cucamonga School District will be considered null and void.

 

KINDERGARTEN

Children who are 5 years of age before September 1st of the year he/she enrolls.
Children who turn 5 between September 2 and June 2 (24-25 SY) are eligible to register for Transitional Kindergarten.
